FEES AND PAYMENTS
To help keep costs down, we ask that you pay for services or any deductibles at the time service is rendered.  Co-payments are required at the front window at check-in.  If surgery is a part of your recommended treatment plan, a payment schedule will be arranged with our Surgery Coordinator.

Please check insurance info for a list of insurance companies with which we participate.  If your insurance company is not on this list, you may be eligible for out-of-network benefits.  Check with your insurance company representative or dial the 800 number on the back of your insurance card.
